USAGov says that depending on your state's rules, you'll find instructions to register or change your information online, by mail, by phone, or in person at your local election office. Your state may "Ask you to re-register to make the information change" or "Provide a special form to use for reporting the change."
The U.S. Election Assistance Commission: "The National Mail Voter Registration Form can be used to register U.S. citizens to vote, to update registration information due to a change of name, make a change of address or to register with a political party."
