Start with the decree
USAGov: "When you file for divorce, you can change your name back to the name you used before you got married. In most states, you may do so when you apply for your divorce decree." The process varies by state; USAGov says to contact your local government to learn how it works where you live.
Get certified copies of the decree. SSA: "You must present original documents or copies certified by the agency that issued them."
Then the same agencies as after marriage
Social Security: SSA lists a "divorce decree" among the documents it may accept to prove a legal name change (SSA). SSA may also need an identity document in your prior name "if you changed your name more than two years ago". See the Social Security steps.
Passport: the DS-82 renewal form accepts a "divorce decree" as proof of the name change; see which passport form fits.
Taxes: the IRS says to "check that both your name and SSN agree with your Social Security card to prevent any delays in processing your return and issuing any refunds" (IRS).
Voter registration, the Post Office and accounts: the same as after marriage (full order).
